Penn State Requirements
As part of your pre-matriculation requirements, you will need to comply with immunization requirements. While incoming students previously received this form in the mail, students now access a secure website (myUHS) and record immunization and health history information online. New students will receive information with instructions on accessing this site after your arrival at Penn State.
- Print out a sample of the online health history form.
- Print out a sample of the online immunization form.
DO NOT MAIL SAMPLE FORMS TO UHS/PENN STATE. YOU MUST ENTER THIS INFORMATION THROUGH myUHS.
Required Immunizations
Please note that you cannot register for second semester classes until you have complied with the Penn State immunization requirement. We do not want your academic endeavors interrupted, so please follow the directions on this site.
Penn State policy requires full time, degree-seeking students accepted to Penn State to provide documentation of:
- immunization against Measles, Mumps and Rubella (MMR)
- Tuberculosis (TB) testing for foreign born students at high risk for TB
- students living in University-owned housing are required by the Commonwealth of Pennsylvania to either be immunized against meningococcal disease or complete a waiver of exemption.
Exceptions
The following students are exempt from the Pre-Matriculation requirements: provisional or non-degree students, and students who are enrolled in World Campus and distance education programs.
Measles, Mumps and Rubella (MMR)
In order to assure immunity the following doses are required:
- Measles: two doses of Measles (Rubella) vaccine
- Mumps: two doses of Mumps vaccine
- Rubella: one dose of Rubella vaccine
The first vaccine must be given after 12 months of age. The second vaccine must be given at least one month after the first vaccine. The cost for receiving the MMR vaccination at UHS plus injection fee is listed in our fee index.
Tuberculosis (TB) testing is required only of students at high risk for TB (as defined by the American College Health Association).

Meningococcal Meningitis
Pennsylvania state law requires all students living in University-owned housing must complete the Meningococcal Vaccine Certification form. Request for exemption for the Meningococcal vaccine must be submitted through Penn State Housing
Help for International Students with Immunization Requirements
UHS is available to provide needed immunizations at the International Student Orientations in August and January each year. Students may also come to the Student Health Center to get the immunizations. Nurse appointment can be made by calling 863.0774 or through myUHS.
Fees
The cost of some of the immunizations (but not all) may be covered for students who have purchased the Penn State Student Health Insurance Plan.
