All incoming undergraduate and graduate students must submit proof of immunizations prior to their arrival on campus.

Visit the Immunizations page for details on required immunizations and the submission process.

Some students, such as student-athletes or students in certain health-related majors may be asked to submit immunization information. These students must also submit proof of immunizations

    Immunization Waivers

    Immunization waivers are required by students who wish to exempt themselves from required vaccinations for medical, religious, or philosophical objections.

    Please note: A student who has been exempt from a vaccination is still considered susceptible to the disease(s) for which the vaccine offers protection. For the safety of the campus community, the student will be subject to removal from the University if the University and/or state public authority advises removal due to a communicable disease outbreak.

    Please note: If you were admitted spring 2020 or earlier, you were required to submit the Penn State Immunization Verification Form, which is no longer required. Please check out how you can submit your official records on our webpage.
